Various forms are available at the back of both churches. Alternatively, you can complete and submit forms online. When submitting forms, please be aware of our full privacy policy.

Baptisms:

Please complete a Baptism Application Form at the backs of each church and return to the Parish Office in person, or post through the Presbytery door (St. Teresa’s). You will need to complete the ‘Baptism Preparation Course’ which is usually on the first Saturday of each month, at 3pm, in the ‘Our Lady Chapel’ (a room to the right of the Altar at St. Teresa’s). The session lasts for approximately 1 hour. Your ‘Rite of Welcome’ Mass and Baptism can then be booked. Baptisms are usually on Saturdays, at 1pm, at St. Teresa’s.
