Catholic Certificate of Practice

The form states that the child and their family are a practicing Catholic family. The definition of ‘practicing Catholic’ is that the family attends Mass each week and that they attend Mass on Holy Days of Obligation.
If you fulfill the criteria, please collect a Catholic Certificate of Practice Application Form, available at the back of each church. Please complete it and put it through the Presbytery door. You will need to book an appointment with Fr. Jude. For the appointment, you should bring your child, their Baptism Certificate, their First Holy Communion Certificate (if applicable) and their Mass Attendance Card (which has been stamped, each week). Appointments can be booked via the Parish Office. This process, and the need for Mass Attendance Cards, has been widely advertised including in newsletters, on notice boards and via the notices announced at the end of Masses. If you attend church weekly, this process should not come as a surprise to you.
